A few things to know about Kaiser Permanente Small Group Plans:

In 2017 Kaiser Permanente acquired Group Health Cooperative and now Kaiser serves more than 11 million members in eight states.  With Kaiser you have a choice between two provider Networks: Core (limited network, traditional HMO) and Access PPO (nearly every provider).

 

ACCESS PPO 
Hundreds of thousands of providers nationwide, including:
• Kaiser Permanente medical offices and pharmacies
• Most providers and designated pharmacies in Kaiser service area,
including UW Medicine, Swedish Physicians, MultiCare, CHI Franciscan, PeaceHealth, Providence and more
• First Choice Health network providers
• OptumRx network pharmacies

CORE
Thousands of quality providers and facilities, including:
• More than 1,000 providers at Kaiser Permanente medical offices*
• 25 Kaiser Permanente medical offices and pharmacies*
• More than 9,000 additional network providers and facilities*

Prescriptions - Formulary List  

A formulary is a list of prescription drugs that are covered under your benefit plan.  With Kaiser group plans (2-50), medications are organized into four levels, or tiers. What you pay depends on which tier your medication is in.  Refer to benefit summary for benefit details for each tier.

Find a Doctor/Hospital - Search

     1) Update your location (zip)

     2) Choose your network:  Access PPO (large network) or Core (limited network)

     3)  Search by provider name
